Graduating seniors: all paperwork must be in our office no later than May 15 in order to participate in the graduation ceremony.

Specialized Education Plan (IEP)

If your student has an IEP from their previous school, use the SEP Progress Report to report their progress in school.

How to Submit

Email all completed forms to paperwork@revivalhomeschool.tv.

• Forms must be filled out using Microsoft Word
• Save each form to your homeschooling folder before filling it in, and save each subject with a different file name
• Print a copy of your paperwork at home for your records
